Артериовенозная мальформация головного мозга (АВМ)

Артериовенозная мальформация – это патологическое скопление извитых кровеносных сосудов в головном или спинном мозге. Тогда как в норме сосуды переносят по организму кровь, богатую кислородом и питательными веществами, в сосудах артериовенозных мальформаций содержится бедная данными элементами кровь.

Большинство исследователей полагают, что практически все артериовенозные мальформации являются врожденными, то есть уже присутствуют в организме ребенка на момент его рождения. В связи с этим можно говорить об артериовенозном врожденном пороке (АВВП). Однако в настоящее время мальформация сосудов не считается наследственным заболеванием.

Артериовенозная мальформация головного мозга

В нормальных условиях кровь поступает в ткани и клетки по артериям, которые разветвляются на мелкие капилляры. Затем кровь возвращается к сердцу по венам. В случае артериовенозной мальформации сосудов кровь в головной или спинной мозг напрямую поступает по артериям в вены, минуя капилляры, что формирует короткий замкнутый цикл кровотока, или шунт.

Симптомы артериовенозной мальформации сосудов

Как правило, АВМ не вызывает какие-либо яркие симптомы. В некоторых ситуациях обнаружение артериовенозной мальформации сосудов головного мозга является случайным, в ходе обследования по поводу других заболеваний или нарушений. Симптомы АВМ в большой степени зависят от локализации патологического роста сосудов. В редких случаях артериовенозная мальформация вызывает эпилептические приступы, неврологические нарушения или боли.

Артериовенозный врожденный порок грозит тяжелыми осложнениями, такими как инсульт, неврологические расстройства. Артериовенозная мальформация сосудов головного мозга может сопровождаться головными болями и эпилептоидными приступами, а также другими специфическими симптомами, которые обычно зависят от локализации АВМ и других факторов.

Лечение артериовенозной мальформации головного мозга

В редких ситуациях, например в случае пожилых пациентов, наилучшим вариантом поведения при мальформации сосудов является обычное наблюдение, без лечения. Практически во всех остальных случаях выделяют три варианта лечения артериовенозной мальформации:

  • хирургическое удаление патологических сосудов,
  • эндоваскулярная (внутрисосудистая) терапия и
  • стереотаксическая хирургия мальформации.

Хирургическое лечение артериовенозной мальформации

В том случае, если в ходе хирургического вмешательства возможно полноценное удаление артериовенозной мальформации сосудов в головном или спинном мозге, то такой вариант лечения является оптимальным. Тем не менее, операции на головном мозге сопровождаются высокими рисками, включая риск инсульта, развития инфекции, неврологических нарушений и осложнений после наркоза.

В ходе эндоваскулярной терапии для получения изображений артериовенозной мальформации проводится специальное исследование под названием ангиография. При этом в сосуды через катетер небольшого диаметра вводится контрастное вещество, после чего проводится рентгеновский снимок. Затем в область артериовенозной мальформации сосудов через катетер вводится особое клейкое вещество. Данная методика менее травматична для пациента, чем открытая операция, однако не всегда позволяет излечить АВМ.

Частично сохранившаяся артериовенозная мальформация сосудов все еще может кровоточить и послужить причиной инсульта. В этой связи большинство нейрохирургов считают, что частичная эмболизация артериовенозной мальформации, то есть частичное закрытие ее просвета, оставляет риск развития осложнений в будущем. В настоящее время внутрисосудистая терапия используется, в основном, перед хирургическим удалением или стереотаксическим вмешательством для уменьшения размеров артериовенозной мальформации спинного или головного мозга.

Стереотаксическая радиохирургия артериовенозной мальформации

В ходе стереотаксического радиохирургического вмешательства к артериовенозной мальформации сосудов подводится рентгеновское излучение под самыми разными углами. С течением времени воздействие излучения вызывает уменьшение просвета сосудов артериовенозной мальформации и, в конечном итоге, их полное закрытие, что устраняет риск кровотечения или инсульта.

Основным преимуществом радиохирургии перед открытым хирургическим вмешательством является малая травматичность для пациентов и отсутствие рисков открытой операции. Более того, некоторые артериовенозные мальформации головного мозга расположены в таких зонах, добраться до которых с помощью обычной хирургии невозможно. В таких случаях радиохирургическое лечение является единственным выполнимым вариантом.

Стереотаксическая радиохирургия может быть использована совместно с другими методами лечения артериовенозной мальформации сосудов головного или спинного мозга. Например, радиохирургия применяется для устранения остаточных очагов артериовенозной мальформации, которые нельзя было удалить в ходе обычной операции. Стереотаксическое радиохирургическое лечение проводится амбулаторно и позволяет пациенту быстро вернуться к привычной повседневной жизни.

Тем не менее, радиохирургия при артериовенозных мальформациях имеет некоторые недостатки. Во-первых, для полноценного устранения мальформации под воздействием излучения может потребоваться до трех лет, в течение которых сохраняется риск кровотечения сосудов. Также возможно развитие серьезных неврологических нарушений в результате кровотечения на фоне артериовенозной мальформации уже после радиохирургического лечения, даже в том случае, когда уменьшение просвета сосудов в ней уже началось. Во-вторых, не все артериовенозные мальформации закрываются после стереотаксического радиохирургического вмешательства.

Вероятность полного исчезновения артериовенозных мальформаций небольшого диаметра (3 см и менее) после однократной процедуры облучения составляет 80%. Коэффициент исчезновения более крупных сосудистых нарушений - ниже. В результате этого нередко после радиохирургического лечения закрывается лишь небольшой участок АВМ. В таких случаях пациентам обычно требуется второй курс стереотаксической радиохирургии.

И, наконец, даже если радиохирургия позволяет полностью излечить артериовенозную мальформацию головного мозга, ее симптомы могут сохраняться. У некоторых пациентов, которые страдали головными болями или судорогами до лечения артериовенозной мальформации, данные симптомы могут сохраняться и после ее полного исчезновения. Считается, что это обусловлено скоплением рубцовой ткани в области артериовенозной мальформации на фоне ее формирования, а также в ходе самого процесса уничтожения патологических сосудов.

Система TrueBeam Stx кардинально отличается от других радиохирургических систем, поскольку не требует использования жесткой металлической фиксирующей рамки, которая прикручивается к костям черепа пациента для иммобилизации и нацеливания на патологический очаг. Высокая точность при использовании системы TrueBeam Stx достигается путем непрерывной компьютерной визуализации, что позволяет постоянно отслеживать положение очага сосудов мальформациии, следовательно, исключает необходимость применения металлической рамки.

Более того, использование других радиохирургических систем нередко ограничено лечением только артериовенозных мальформаций сосудов головного мозга. В то же время система TruBeam Stx применяется и в случае лечения артериовенозных мальформаций и спинного мозга.

Иногда наилучшим решением для излечения от мальформации сосудов является сочетание нескольких (или даже всех) методов терапии, для достижения наилучшего результата с минимизацией риска побочных эффектов.
